It’s not unusual for consumers to use well-known products such as such as Alka-Seltzer Original, Bromo Seltzer, and Zee-Seltzer Antacid and Pain Reliever, to treat an upset stomach, heartburn or indigestion. The FDA is now warning that serious internal bleeding can occur after using medicines with an antacid, such as sodium bicarbonate, that neutralizes stomach acid with aspirin.

Aspirin, a commonly used pain reducer and fever reducer, is a nonsteroidal anti-inflammatory drug (NSAID). Besides aspirin, NSAIDs also include ibuprofen, naproxen, and ketoprofen. In 2009, the FDA warned that NSAIDs can increase the risk of bleeding, including in the stomach and gastrointestinal tract. The warning signs of internal bleeding include abdominal pain, feeling faint, vomiting blood, and passing black or bloody stools. At the time, the FDA told makers of over-the-counter and prescription NSAID products to carry a warning on their products’ safety label of bleeding risks. However, since 2009, the FDA said it had received eight new reports of serious bleeding caused by antacids that contain aspirin.

According to the FDA, people have a higher risk of stomach bleeding if they are over 60, have a history of stomach ulcers or bleeding problems, take blood thinners or are heavy drinkers. The FDA noted the eight cases of severe bleeding reported since 2009 had underlying conditions such as these risk factors that placed the patients at greater risk for developing serious bleeding events. The FDA said it only reviewed cases submitted to its database, so there are more than likely other unreported cases.

In addition to aspirin-related bleeding risks, NSAIDs can increase the chance of other life-threatening injuries including heart attacks and strokes. In July 2015, the FDA required manufacturers of prescription and over-the-counter NSAIDs to place boxed warnings on their product labels regarding the health risks associated with NSAIDs. Over-the-counter NSAIDs, such as ibuprofen (Motrin, Advil) and naproxen (Aleve), are used to temporarily reduce fever and treat minor aches and pains.
